Clinical Lead – Hospital at Home Service

Are you an experienced clinical leader passionate about delivering high-quality, patient-centred care in the community? Our client is looking for a dynamic Clinical Lead to oversee and drive the development of their innovative Hospital at Home service, delivering acute, reablement and discharge support to patients in the comfort of their own homes.

This is a unique opportunity to combine hands-on clinical expertise with service development, leadership and operational responsibility—shaping the future of community-based care pathways including Virtual Wards, Early Supported Discharge and Admission Avoidance.


About the Role

As Clinical Lead, you will be responsible for ensuring the safe, effective and high-quality delivery of care across our client’s expanding Hospital at Home services. Working closely with the CEO, you will support the growth of the service, contribute to commercial opportunities and lead a multidisciplinary team delivering integrated care.

You’ll operationally manage key pathways, oversee virtual ward packages, maintain clinical governance standards and ensure the service meets performance, quality and regulatory requirements.


Key Responsibilities

Clinical Leadership & Delivery

  • Provide high-quality nursing care across acute, reablement and discharge pathways.

  • Undertake assessments, develop care plans and monitor patient outcomes at home.

  • Lead MDT collaboration to deliver integrated, patient-centred care.

  • Maintain accurate clinical documentation and ensure regulatory compliance.

  • Manage caseloads, workforce planning and equitable workload distribution.

  • Support virtual ward operations, case finding and patient flow.

  • Maintain clinical competencies and act as a visible clinical role model.

Operational & Service Management

  • Lead the day-to-day operational management of the service.

  • Manage KPIs, performance targets and clinical standards.

  • Oversee service budgets and ensure efficient use of resources.

  • Manage incidents, complaints and quality improvement initiatives.

  • Support recruitment, induction, supervision and performance reviews for staff.

Quality, Governance & Safety

  • Ensure robust clinical governance, risk management and audit processes.

  • Maintain a safe working environment for patients, staff and visitors.

  • Ensure all staff complete mandatory training and follow policies.

  • Champion high standards of infection control, medicines management and safeguarding.

Service & Commercial Development

  • Support the CEO with identifying and developing commercial opportunities.

  • Contribute to service design, transformation, and partnership development.

  • Promote the services to external stakeholders and NHS partners.

  • Support outcome reporting, data analysis and development of bespoke care packages.


About You

Essential Requirements

  • Registered Nurse (Adult) with current NMC registration.

  • Minimum 5 years post-registration experience.

  • Strong background in community nursing, urgent care or hospital discharge.

  • Experience in staff management, service improvement and achieving KPIs.

  • Ability to analyse data and present complex information.

  • Confident decision-maker with excellent communication skills.

  • Digital literacy, including MS Office and clinical systems.

  • Full UK driving licence and access to a vehicle (business insurance required).

Desirable

  • Experience with Virtual Wards, Hospital at Home, OPAT or remote monitoring.

  • Knowledge of CQC/CIW standards.

  • Mentorship qualification or postgraduate study.

  • Experience with service transformation, contracting or business planning.
